• Choose your sport and provide all the required information for each field. 
• Pay close attention to spelling and capitalization.
• Upload your team logo if applicable 

A proof will be emailed to you for review and approval, your order will not be worked on until approval is received. Once approved it will be placed into production and we will not accept any further changes. 

IMPORTANT: If changes are requested after approval has been received you will be charged the full price of an additional ornament.
